Appointments

How do I schedule an appointment?

You can schedule an appointment by:

  • Calling our office AT 404-256-0775
  • Requesting an appointment via the patient portal

What should I bring to my appointment?

Please bring:

  • A valid photo ID
  • Your insurance card
  • Completed Intake Paperwork
  • A list of medications you are currently taking
  • Any relevant medical records or referrals

What if I need to cancel or reschedule?

Please notify us at least 24 hours before your appointment if you need to cancel or reschedule. Cancellation within 24 hours of your appointment will result in a $35-dollar late cancellation fee.

What happens if I arrive late?

We offer a 15 minute grace period. If you arrive late, we will do our best to accommodate you. However, you may need to reschedule if the provider’s schedule cannot accommodate a late arrival.


Insurance & Payments

What insurance plans do you accept?

We accept many major insurance plans. Please contact our office or check our website for the most up-to-date list. We also encourage you to contact your insurance company directly for coverage details.

Will I need to pay a copay?

Yes. Copays, deductibles, or other patient responsibilities are typically collected at the time of your visit.

What if I don’t have insurance?

Please call our office for information regarding out-of-pocket costs.

Who should I contact for billing questions?

If you have questions about your bill, please contact our billing department at 404-256-0775 (Prompt #3) or send a message through the patient portal.


Prescriptions

How do I request a prescription refill?

The fastest way to request a refill is to:

  • Submit a request through the patient portal, or
  • Contact your pharmacy, which will send us a refill request.

Please allow 24–48 hours for refill processing.


Lab Results

How will I receive my test results?

Your results may be shared through:

  • The patient portal
  • A phone call from your doctor or medical assistant.
  • A follow-up appointment, if needed.

How long do results take?

Most results are available within 2–5 business days, depending on the test.


Medical Records

How can I request a copy of my medical records?

You may request your records by completing a medical records authorization form. Please contact the office for assistance with obtaining this form.

Can my records be sent to another doctor?

Yes. With your written authorization, we can send your records to another healthcare provider.

Telehealth Visits

Do you offer telehealth appointments?

Yes. Telehealth appointments may be available for certain types of visits, such as follow-ups or medication reviews.

What do I need for a telehealth visit?

You will need:

  • A smartphone, tablet, or computer
  • Internet access
  • A quiet, private space

Through the portal you can:

  • Schedule appointments

What if I forgot my portal login?

You can reset your password online from the portal login page or contact our office for assistance.
